Bug Fix - Unable to add two of the same clinical codes

What have we done?

Changed the AI coding process to allow duplicate clinical codes to be added for different event dates.

Why have we done this?

To allow the system to add the same clinical code for a patient, on a different date. E.g. A scan with clinical code XXX may be done on date 1 and date 2

How will you benefit?

Users will be add the same clinical codes on different date.

Details of the change

Update the code to allow the same clinical code on different dates.
